Please print out paper copies, answer the questions, and bring the completed forms with you to your first appointment at the clinic.
All New Patients are required to fill out the "New Patient Questionnaire," the "Office Policies," and the "Informed Consent" forms. All female patients are also required to fill out the "Female Questionnaire."
